STATUTORY DECLARATION

STATUTORY DECLARATION AFFIDAVITS AND THEIR — USE TO DOCUMENT
TIME WORKED AS AN ELECTRICIAN

An affidavit may be used to document time worked as an electrician when applying to challenge a Certificate of Qualification examination in the following areas:

1.    when the firm is no longer in business and the principals cannot be located
2.    when the owner/manager has died and complete records are not available
3.    when the firm is located overseas and extreme difficulties are encountered in trying to get documentation
4.    where the applicant has been self-employed as an owner/operator of a business
5.    when a firm refuses to issue a letter to document time worked in a trade

An affidavit may not be used as a substitute when a letter from an employer could be obtained within a reasonable time and without too much difficulty or expense.

A Notary Public or Lawyer should draw up an affidavit on the proper form that they have for such occasions, and must contain the following information:

1.    name and address of the employer(s) you worked for
2.    the year and month you started with the employer(s)
3.     the year and month you terminated with the employer(s)
4.    the trade in which you worked for the employer(s)
5.    a brief description of your duties while working for the employer(s)
6.    the reason why an employer letter is unavailable
7.    a statement that the work was full time, and if not, the number of months worked in total for this employer

Attached to the affidavit, depending on the circumstances, must be on of the following:

1.    T4 slips for employment in Canada for the dates indicated on the affidavit
2.    copy of landed papers for any Landed Immigrants
3.    business license or a list of suppliers in the case of a sole proprietor

 

Affidavits are not acceptable for the applicant if any of the above information is missing.
